Lifecare Homecare is seeking an Occupational Therapist to provide in-home assessments and interventions across Victoria. You will work within a multidisciplinary team to support independent living and quality care.

Qualifications

• Occupational Therapist qualification with General AHPRA registration.
• Experience delivering in-home assessments and interventions.
• Ability to collaborate in a multi-disciplinary team.
• Current Working with Children Check and NDIS Clearance or ability to obtain.

Responsibilities

• Deliver high-quality OT assessments and interventions in clients' homes.
• Tailor services to individual client needs using person-centred approaches.
• Collaborate with Case Managers, GPs and other health professionals.
• Prepare reports to meet funding body standards (NDIS, DVA, Medicare).
• Reflect Lifecare values of Care, Respect, Integrity and Service Excellence.
• Build relationships with referrers and community partners.

Job description

As a full time or part time Occupational Therapist you will be providing high-quality assessments and interventions to our clients to support independent living. This encompasses ensuring effective and efficient assessment, planning, interventions and evaluation in a multi-disciplinary setting.

In this role you will be…

• Ensuring appropriate delivery of quality Occupational Therapy services tailored to the individual needs of the client and using a person-centred approach
• Conducting assessments and providing interventions to clients in their own home
• Engaging with other Case Managers, Allied Health Professionals, GP’s and other Health Care Professionals to provide effective client care
• Writing reports which meet the standards of funding bodies such as NDIS, DVA, Medicare and HCP Providers
• Able to reflect our practice values of Care, Respect, Integrity and Service Excellence in all that you do.
• Creating and maintaining effective relationships with Case Managers, GP’s, community centre’s and other referrers.
