Job summary

Lifecare Homecare, part of the Lifecare network within the APM Group, is seeking a Physiotherapist to deliver high-quality home-based care across Western Australia. You will assess, plan, implement and evaluate treatment in clients' homes, and work within a multidisciplinary team to achieve optimal health outcomes.

You will hold a Physiotherapy degree with General AHPRA registration, demonstrate the ability to work autonomously in the community and with older adults and disability groups, and

Qualifications

• Physiotherapist with General AHPRA registration.
• Ability to work in a multi-disciplinary team and autonomously in the community.
• Experience with older adults and adult disability demographics preferred.

Responsibilities

• Ensuring appropriate delivery of quality Physiotherapy services tailored to individual client needs in the home.
• Conducting assessments and providing interventions in clients' own homes.
• Engaging with other Allied Health Professionals, GPs and health care professionals to coordinate care.
• Writing initial, progress, and final reports and treatment management plans compliant with funding bodies (NDIS, DVA, Medicare, HCP).
• Reflecting Lifecare values of Care, Respect, Integrity and Service Excellence in all activities.
• Creating and maintaining effective relationships with Case Managers, GPs, community centres and referrers.

About the role

As a full time or part time Physiotherapist you will be providing high-quality treatment to our clients to ensure the provision of optimal health outcomes. This encompasses ensuring effective and efficient assessment, treatment planning, treatment implementation and evaluation in a multi-disciplinary setting.

In this role you will be…

• Engaging with other Allied Health Professionals, GP’s and other Health Care Professionals to provide effective patient care
• Writing reports such as initial reports, progress and final reports and treatment management plans which meet the standards of funding bodies such as NDIS, DVA, Medicare and HCP Providers
• Able to reflect our practice values of Care, Respect, Integrity and Service Excellence in all that you do.
• Creating and maintaining effective relationships with Case Managers, GP’s, community centre’s and other referrers.
