Job summary

Bathurst Health Service is seeking a Nurse Practitioner/Transitional Nurse Practitioner to deliver advanced clinical assessment, diagnosis, treatment and care coordination for older adults in Bathurst’s community aged care setting. The role supports preventing avoidable hospital presentations and works closely with Emergency Department, Palliative Care, HITH and inpatient aged care teams.

The position is permanent full-time with 38 hours per week.

Position Classification: Nurse Practitioner / Transitional Nurse Practitioner (TNP will be under a maximum of 3-year contract paid at Clinical Nurse Consultant Grade 2 whilst undertaking requirements to become an Endorsed Nurse Practitioner. On endorsement, the position holder will transition to NP Year 1).
Remuneration: $149,408.12 - $169,429.03 pa (+ superannuation + salary packaging options)

About the role
Join our innovative aged care team as an Aged Care Nurse Practitioner/Transitional Nurse Practitioner (TNP) in Bathurst. This exciting community-based role provides advanced clinical assessment, diagnosis, treatment, and care coordination for older people, helping them remain safely in their homes and communities.

With a strong focus on preventing avoidable hospital presentations and admissions, you will work closely with Emergency Department, Palliative Care, HITH, and inpatient aged care teams to deliver timely, high-quality care and improve patient outcomes across the continuum of care.
