Job summary

Bathurst Health Service is seeking a Nurse Practitioner/Transitional Nurse Practitioner to deliver advanced clinical assessment, diagnosis, treatment and care coordination for older adults in Bathurst’s community aged care setting. The role supports preventing avoidable hospital presentations and works closely with Emergency Department, Palliative Care, HITH and inpatient aged care teams.

The position is permanent full-time with 38 hours per week.
