Job summary

Western NSW Local Health District is seeking a Nurse Practitioner / Transitional Nurse Practitioner (TNP) in Bathurst to deliver advanced clinical assessment, diagnosis, and care coordination for older people in the community.

The role focuses on preventing avoidable hospital presentations and works closely with Emergency Department, Palliative Care, HITH and inpatient aged care teams to improve patient outcomes across the continuum of care.

Qualifications

• Advanced clinical assessment, diagnosis, treatment and care coordination for older people.
• Experience in community aged care and collaboration with ED, Palliative Care, HITH.
• Endorsement as Nurse Practitioner; TNP to become Endorsed NP within 3-year contract.

Responsibilities

• Deliver high-quality, timely care to older adults in community settings.
• Collaborate with ED, Palliative Care, HITH and inpatient teams to prevent hospital admissions.
• Support care planning, diagnosis, and treatment in line with NP scope.

Position Classification: Nurse Practitioner / Transitional Nurse Practitioner (TNP will be under a maximum of 3-year contract paid at Clinical Nurse Consultant Grade 2 whilst undertaking requirements to become an Endorsed Nurse Practitioner. On endorsement, the position holder will transition to NP Year 1).
