Job summary

Hunter Primary Care (HPC) seeks Endorsed Nurse Practitioners to provide compassionate, evidence-based primary health care to older adults living in Residential Aged Care Homes. You will work autonomously, with mentorship from an experienced NP team, and collaborate with multidisciplinary colleagues to achieve optimal health outcomes.

You will hold NP endorsement, APHRA registration, and meet regulatory requirements including NDIS clearance and police checks.

Qualifications

• Must be endorsed by the Nursing and Midwifery Board of Australia to practice as a Nurse Practitioner.
• Current APHRA registration as a Nurse Practitioner is required.
• CPR certification is required.
• NDIS Worker Clearance and National Police Check are required.

Responsibilities

• Deliver evidence-based, consumer-centred care in aged care settings, autonomously and with the team.
• Prescribe and deprescribe medications in line with best practices and regulatory guidelines.
• Collaborate with General Practitioners and the clinical services team to optimise resident health outcomes.
• Assist with disease prevention, health promotion, and chronic disease management.

The HPC Nurse Practitioner (NP) service is dedicated in providing high-quality, compassionate healthcare to older adults living in Residential Aged Care Homes (RACHs). Our focus is on delivering holistic, patient-centred care that enhances the quality of life for all residents.

Position Overview:

As an established provider in the community, we are looking for Endorsed Nurse Practitioners in the Manning, Tamworth, Newcastle/Central Coast regions to provide compassionate, comprehensive, evidence-based primary health care to older persons living in the RACH setting. You will be fully supported by our experienced NP team and will have access to mentorship in a collaborative work environment. You will work autonomously and closely with multidisciplinary teams to ensure optimal health outcomes, focusing on disease prevention, health promotion, and chronic disease management. Essential skills, experience and responsibilities:

• Currently registered as a registered nurse with the Australian Health Professionals Regulation Agency (APHRA) and endorsed by the Nursing and Midwifery Board of Australia to practice as a Nurse Practitioner
• Ability to deliver evidenced based, consumer-centred care within the aged care environment, autonomously and collaboratively, using advanced and extended clinical skills.
• Prescribe (& deprescribe) medications and manage treatment plans in line with best practices and regulatory guidelines.
• Demonstrated knowledge and understanding of regulatory compliance governing the Aged Care environment
• Highly developed communication skills with a demonstrated ability to manage successful working relationships with residents, families, General Practitioners and the clinical services team.
• Demonstrated effective time and schedule management to ensure residents expectations and health care goals are met
• High-level computer skills

The successful applicant will be required to hold:

• NDIS Workers Clearance
• Current CPR certification.
• National Police Check Clearance.
• Professional Indemnity Insurance
