Job summary

Wheatfields Incorporated in Freeling, SA, is expanding to 72 residents and seeks a Nurse Practitioner to join our clinical leadership team, delivering high-quality, person-centred healthcare.

The NP will collaborate with GPs, Clinical Manager, RNs and allied health to provide advanced assessment, diagnosis, treatment and ongoing management of residents with complex healthcare needs, improving access to medical care and strengthening governance.

Qualifications

• Current general registration as a Registered Nurse with AHPRA.
• Current endorsement as a Nurse Practitioner.
• Experience in aged care, primary healthcare or gerontology.
• Postgraduate qualifications or advanced experience in gerontology, palliative care or primary healthcare.
• Knowledge of contemporary medication management and prescribing practices.

Responsibilities

• Advanced Clinical Practice: undertake comprehensive assessments, diagnose and develop evidence-based plans.
• Prescribe, review and manage medications within NP scope and legislative requirements.
• Request and interpret diagnostic investigations and initiate treatment.
• Manage acute and chronic conditions, frailty and complex comorbidities.
• Provide advanced wound, pain and continence management and medication optimisation.
• Facilitate timely referrals to specialists, hospitals and community health services.
