An exciting opportunity exists for a highly skilled Nurse Practitioner (NP)or Transitional Nurse Practitioner (TNP)with advanced clinical expertise in Palliative Careand a strong interest in Aged Careto join the Palliative Aged Care Supportive Service (PACSS).
In this role, you will deliver advanced nursing assessment, diagnosis, and clinical care to patients and their families within a defined scope of practice, consistent with the NSW Health Nurse Practitioner Frameworkand relevant professional standards. You will also provide clinical leadership, contribute to strategic service development, and support governance processes to strengthen models of care for residents transitioning to end-of-life care.
About the Role
As the NP/TNP within PACSS, you will provide expert palliative care to older people residing in Residential Aged Care Facilities (RACFs)across the Northern Beaches catchment.
Working collaboratively with the NSLHD Specialist Palliative Care Service, Beaches Rapid Access Care of the Elderly (BRACE), General Practitioners, and HammondCare Palliative Care, you will:
