Job summary

Lumia Care is urgently seeking compassionate Disability Community Support Workers in the South West Region, NSW, to help clients live safely and independently at home while staying connected to their community. You will provide personal care, assist with mobility, support domestic tasks, monitor wellbeing, and follow care plans with clear communication and empathy.

We're urgently looking for compassionate Disability Community Support Workers living in the South West Region. In this rewarding role, you'll provide support to clients, helping them live safely and independently in their own homes while staying connected to their community. You'll provide ongoing assistance, monitor clients' wellbeing, and respond promptly to their care needs.

What You'll Be Doing

• Provide personal care support safely and respectfully
• Assist with mobility, transfers, and wheelchair management if needed
• Support domestic tasks and home organisation
• Provide emotional support and maintain a calm, structured environment
• Monitor fatigue, pain levels, and signs of distress
• Follow care plan, risk management strategies, and reporting requirements
