Job description

Non-Clinical Care Partner – Support at Home

Absolute Care and Health – South Yarra VIC

Absolute Care & Health | South Yarra | Full Time

Hello there,

Are you beautifully organised, confident working across systems and someone who likes making sure all the details are exactly where they need to be?

Can you keep care plans, service arrangements, budgets and client records on track, while calmly following up the many moving parts behind the scenes?

Lovely. We should talk.

We’re Absolute Care & Health, and we’re in the business of making care and support an absolutely positive experience. We believe people are happiest in their own home, living life their way, and our team helps make that happen every day.

Our values are not just words on a wall. We aim to:

  • Go above and beyond
  • Create belonging
  • Re-imagine what better could look like
  • Understand people properly, not just process requests.

We’re looking for a Non-Clinical Care Partner to provide practical and administrative support for clients receiving services through the Support at Home program.

This is not a clinical role, and it is more administrative than client-facing. It would suit someone who enjoys coordinating information, maintaining accurate records, monitoring budgets and making sure care arrangements are properly documented and actioned.

So, what will you actually be doing?

You’ll help keep the administrative side of each client’s care organised, accurate and moving in the right direction.

  • Preparing, updating and reviewing the non-clinical components of client care plans
  • Coordinating services in line with approved care plans, client preferences and available funding
  • Maintaining accurate client records, contact notes and service information
  • Monitoring client budgets, service expenditure and funding allocations
  • Helping ensure planned services remain within the client’s approved funding
  • Preparing information to help clients and their representatives understand service arrangements and costs
  • Processing changes to services and following up outstanding actions
  • Coordinating information between care coordination, clinical, administration, service delivery and finance teams
  • Liaising with external service providers to organise or confirm services
  • Identifying gaps, errors or inconsistencies in client information and following them through to resolution
  • Keeping records and documentation up to date and compliant with organisational and Support at Home requirements
  • Supporting care plan reviews and other scheduled client reviews
  • Escalating changes in a client’s health or circumstances to the Clinical Care Partner or Anchor Registered Nurse
  • Contributing to audits, quality reviews and service improvement activities.

In short: you’ll help make sure the plans are current, the services are coordinated, the budgets are monitored and the records are accurate.

No cape required. But a love of detail, good systems and following things through will be very useful.

You’ll probably love this role if you…

  • Are beautifully organised and secretly enjoy a good spreadsheet, system or checklist
  • Like keeping records accurate, complete and up to date
  • Can manage multiple tasks, deadlines and client files without losing sight of the details
  • Are comfortable working with budgets, service allocations and costs
  • Enjoy solving administrative problems and following up outstanding actions
  • Communicate clearly and professionally by phone and email
  • Are confident working across different systems and databases
  • Know when something needs to be escalated to a clinical or senior team member
  • Work well with operational, clinical, administration and finance teams
  • Take pride in making sure nothing important falls through the cracks
  • Believe good administration is an essential part of delivering good care.

The practical bits

We’d love you to have:

  • Strong administration or service coordination experience
  • Experience working in aged care, community services, disability, healthcare or another service-based environment
  • Experience maintaining client files, care plans, service agreements or similar records
  • Strong attention to detail and confidence working with data and documentation
  • The ability to manage competing priorities and follow matters through to completion
  • Confidence working with budgets, service expenditure or funding allocations
  • Strong written and verbal communication skills
  • Good computer literacy, including Microsoft Office and electronic client management systems
  • A qualification, or studies towards a qualification, in aged care, community services, disability or a related area, or equivalent relevant experience is highly regarded.
  • Experience with the Support at Home program, Home Care Packages, community aged care funding or AlayaCare would be wonderful, but it is not essential if you are a capable administrator who can quickly learn new systems and program requirements.

This is a non-clinical position. Clinical assessments, clinical decisions and clinical care planning will be completed by appropriately qualified members of the Integrated Care Team.

What now?

You must have the right to work in Australia and achieve a satisfactory outcome for any police checks required, complete an NDIS check and NDIS training as part of the recruitment process.
