Gateway Health is seeking a Non-Clinical Aged Care Needs Assessor – Home Support in Wodonga. This temporary part-time role involves conducting face-to-face assessments for access to Commonwealth subsidised aged care services, and facilitating suitable support options to improve independence and wellbeing.

Role operates under the Social and Community Service Employees Multi Enterprise Agreement 2022, level 4, with hourly pay of $46.70–$50.25 plus superannuation; 45.6 hours per fortnight for 12

Non-Clinical Aged Care Needs Assessor – Home Support

About the Role

This is a rewarding temporary part-time opportunity (45.6 hours per fortnight for 12 months) based in Wodonga, where you'll provide high-quality assessments to older people, who are seeking access to Commonwealth subsidised aged care services. This includes assessing individual needs, identifying appropriate support options and facilitating access to services that promote independence, wellbeing and quality of life. Assessments will mainly be conducted face to face, in the client’s home.

This role is classified under the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2022, at level 4 ($46.70 - $50.25 per hour + superannuation).

Skills & Experience

To thrive in this role, you'll bring:

• Demonstrated skills and experience in assessment, referral, case management, carer support or similar.
• Demonstrated knowledge and experience of the aged care service system, including knowledge of contemporary principles and practice in wellness and reablement
• Advanced clinical expertise in continence and/or urology nursing
• Well-developed communication skills with the ability to use active listening techniques, negotiation and influencing skills, and the ability to prepare reports and correspondence in a manner appropriate to the purpose and audience.

A tertiary qualification in a relevant health or social science discipline, such as Aged Care, Enrolled Nursing, Community Services or Allied Health Assistance, will be highly regarded but is not essential.
