Job summary

Uniting Kari Court in St Ives values compassionate, patient-centered aged care. This role leads clinical care planning and service delivery for residents with dementia, guiding a team of carers and coordinating with health professionals to achieve individual goals.

Qualifications

Responsibilities

  • Lead the resident care planning, delivery and evaluation.
  • Provide clinical services with sharp assessment skills.
  • Know residents well and advocate for their goals.
  • Ensure care meets individual goals and promotes quality of life.
  • Liaise with medical and allied health professionals and families to support residents.
  • Supervise and support Personal Care Workers.
  • Lead the team with holistic decision-making.

Permanent shifts on offer:



  • AM shifts - Mon, Tues, Wed - 7am - 3pm

  • Wednesday Night Duty shift - 10:30pm - 7:15am


About Uniting Kari Court

Based in St Ives, Uniting Kari Court is a welcoming, close-knit community made up of four households, each home to up to 14 residents. It provides specialist dementia support to just 55 residents, ensuring truly personalised care in a calm and supportive environment.


Our Household Living approach brings the comfort of a family home to everyday life, empowering residents to live their day, their way. Supported by our dedicated Uniting Homemakers, each household is a vibrant place where meaningful moments, familiar routines and enjoyable activities help residents feel connected, valued and at home.


What you’ll be doing in this role


  • You will be a clinical leader in the service, and you will develop, deliver and evaluate resident care plans to meet the individual needs of our residents

  • You’ll need sharp clinical assessment skills and will take the lead in providing clinical services

  • Get to know each resident and become a fundamental part of their life

  • Play an important role in making sure the care provided meets residents’ individual goals

  • You’ll liaise with medical and allied health professionals and residents’ family members to advocate for clients’ quality of life

  • Supervise and support our Personal Care Workers

  • Practice in a holistic way, using your critical decision-making skills to lead the team.

Registered Nurses who are new to aged care, or those in their first or second year of practice who have not previously completed a Graduate Nurse Program, are encouraged to participate in Uniting’s Aged Care Transition to Practice Program. Who we’re looking for


  • You’ll have previous registered nursing experience in aged or community care, or a passion to begin a career in aged care if you have nursing experience in the wider health sector.

  • You’ll be adaptable, empathetic, resourceful, a great time manager, and be able to live Uniting’s everyday values of being compassionate, respectful, imaginative, and bold.

  • You’ll be committed to contemporary aged care nursing practice.

  • You’ll hold a nursing degree with current AHPRA registration.
