Job summary

Apex Support Service Pty Ltd in Sydney is seeking a proactive Team Leader to oversee high-quality disability supports across the Greater Sydney region. This mobile role blends leadership, participant support, and staff development, reporting to the Operations Team.

You will drive onboarding, care planning, supervision, quality assurance, and compliance with NDIS Practice Standards and Code of Conduct while supporting a dedicated team.

Qualifications

• Diploma or Bachelor's degree in Community Services, Disability, Social Work, Psychology, Human Services, Nursing, or related field.
• Minimum 1–2 years' experience in the disability sector.
• Experience supporting people with physical or complex needs, autism, brain injury, or psychosocial disability.
• Experience developing care plans, participant intakes, and supervising support staff.
• Knowledge of NDIS Practice Standards and NDIS Code of Conduct.
• Competency in medication administration, manual handling, infection control, and mobility equipment.

Responsibilities

• Oversee participant intake meetings and develop care plans.
• Provide ongoing coaching and field-based supervision to Disability Support Workers.
• Assist with rostering, workforce planning, and matching participants to workers.
• Deliver direct participant support (in-home, community, SIL, respite).
• Maintain records, incident reports, and care plan reviews.
• Engage with participants, families, and allied health professionals to meet goals.
