Job summary

Challenge Community Services is seeking a Team Leader - Support Coordination in Dubbo, NSW to lead and supervise a team of Support Coordinators while managing a small caseload of NDIS participants.

You will support participants and families to understand, build, and implement their NDIS plans, covering both Support Coordination and Specialist Support Coordination for complex needs. The role involves coordinating formal, mainstream and funded supports, and strengthening participant networks.

Qualifications

• Experience with NDIS participants and support coordination.
• Understanding of NDIS plans and funded, mainstream and informal supports.
• Ability to coordinate multiple supports and work effectively with participants and their families.
• Ability to support people with complex needs.
• Strong communication, organisation and coordination skills.
• Ability to provide leadership, supervision and support to a team.
• Experience in Specialist Support Coordination.
• Experience leading or supervising a team.
• Experience working within a complex service delivery environment.

Responsibilities

• Provide leadership, supervision and support to Support Coordinators.
• Manage a small caseload of NDIS participants.
• Provide Support Coordination to help participants understand and implement their NDIS plans.
• Coordinate formal, mainstream and funded supports.
• Assist participants to build capacity and resilience within their support networks.
• Provide Specialist Support Coordination for participants with more complex support needs.
• Work collaboratively with participants, families and support networks.
• Support the team to achieve organisational goals.
