Job summary

St Louis Community Care Packages Pty Ltd in South Australia is looking for an experienced Support Worker to join our permanent full-time team. You will provide person-centred care, assist with daily living activities and help clients maintain independence.

You’ll work with a friendly care team, follow individual care plans, document progress and report changes promptly. The role offers a competitive hourly rate and opportunities for professional development within a long-established community

Qualifications

• Certificate III in Individual Support (Ageing and Disability) or equivalent.
• Minimum 1 year of relevant experience.
• Demonstrated ability to provide compassionate, respectful and person-centred care.
• Excellent communication and interpersonal skills.
• Current National Police Clearance.
• Current First Aid and CPR Certificate or willingness to obtain.
• Current Australian Driver Licence and access to a reliable vehicle.
• Current Influenza vaccination and other vaccinations as required.

Responsibilities

• Provide personal care, including assistance with showering, dressing, grooming, toileting and mobility.
• Assist clients with domestic assistance, including cleaning, laundry and maintaining a safe home environment.
• Prepare meals and assist with nutrition in accordance with individual dietary requirements.
• Provide companionship, social support and assistance with community participation; transport clients to medical appointments, shopping and social activities where required.
• Monitor clients' wellbeing and promptly report any changes in health or care needs; maintain accurate records and progress notes.
• Follow individual care plans and comply with aged care standards, WHS requirements and infection control procedures.
• Work collaboratively with coordinators, nurses, allied health professionals and other care team members.

We are seeking experienced and compassionate Support Worker to join our growing team on a permanent full-time basis.

Key Responsibilities

• Work collaboratively with coordinators, nurses, allied health professionals and other members of the care team to achieve positive outcomes for clients.

Essential Requirements

• Certificate III in Individual Support (Ageing and Disability), or an equivalent relevant qualification.
• At least one year of recent highly relevant experience.
• Demonstrated ability to provide compassionate, respectful and person-centred care.
• Excellent communication and interpersonal skills.
• Current National Police Clearance.
• Current First Aid and CPR Certificate (or willingness to obtain).
• Current Australian Driver Licence and access to a reliable, insured motor vehicle.
• Current Influenza vaccination and other vaccinations as required.
