Job summary

Hope and Care Community Services is seeking passionate and reliable Support Workers to join our team across Brisbane and surrounding areas. You will provide person-centred support to participants in their home and community and assist with daily living activities.

Applicants should have a current C Class Driver Licence and access to a registered vehicle, with a focus on promoting dignity, independence and connection to the community.

• Build respectful relationships with participants, their families and support networks.
• Follow care plans and provide support according to participant needs and goals.
• Complete required administrative tasks and documentation.
• Work collaboratively with other team members and communicate effectively with participants and their support networks.
• Maintain a safe, clean and respectful environment.

• Good communication and interpersonal skills.
• A genuine commitment to supporting people with disability and promoting their dignity, independence and rights.
• Experience in disability support, mental health or psychosocial support, youth work, aged care, community services, personal care, or supporting people with complex support needs is highly regarded.
