Job summary

Communify Qld in Bardon, Brisbane is seeking an experienced NDIS Support Coordinator to assist participants with understanding and implementing their NDIS plans, building capacity and connecting with services. This part-time fixed-term role runs until 30 June 2027, with hybrid work after probation.

You'll work independently and as part of a supportive team, applying your knowledge of mental health and psychosocial disability to design flexible supports within the community.

Qualifications

• NDIS knowledge of plans and supports.
• Experience in mental health and psychosocial disability.
• Ability to assess, plan, implement, and evaluate supports.

Responsibilities

• Support participants to understand and implement their NDIS plans.
• Build capacity and connect participants with services and supports.
• Collaborate with service providers to coordinate flexible supports in the community.
