Job summary

ConnectGV is seeking a Casual Disability Support Worker to provide personalised in-home support across the Goulburn Valley. This role focuses on one-on-one care in participants’ homes, with opportunities for community access and participation as required.

• Previous experience in disability support, aged care, community services, health or a related field.
• Certificate III in Individual Support, Disability, Aged Care or an equivalent qualification, or a willingness to obtain one.
• Ability to provide personal care and support with daily living activities.
• Ability to safely administer medication following appropriate training and organisational procedures.
• Strong communication and interpersonal skills.

Responsibilities

• Provide person-centred support based on each participant’s individual needs, goals, preferences and support plan.
• Assist participants with personal care, daily living activities, medication administration and other individual support needs.
• Support participants with meal preparation, laundry, light household duties, grocery shopping, light gardening and maintaining a safe and comfortable home environment.
• Support participants with community access, appointments, social activities and maintaining community connections.
• Promote choice, independence, dignity, privacy and participation in everyday activities.
• Maintain accurate records and follow individual support plans, organisational procedures and safeguarding requirements.
• Communicate effectively with participants, families, colleagues and other relevant stakeholders.
• Identify and report concerns, incidents, risks or changes in a participant’s wellbeing.
• Maintain professional boundaries and contribute to a safe, respectful and inclusive environment.

ConnectGV is growing its Individual Support team, and we are excited to introduce new Casual Disability Support Worker opportunities.

This is a new role focused on providing personalised, one-on-one support in participants’ own homes, with community access and participation also forming part of the role where required.

KEY RESPONSIBILITIES

ABOUT YOU

We are looking for people who are passionate about supporting others and who can demonstrate:

• Previous experience in disability support, aged care, community services, health or a related field.
• Certificate III in Individual Support, Disability, Aged Care or an equivalent qualification, or a willingness to obtain one.
• Ability to provide personal care and support with daily living activities.
• Ability to safely administer medication following appropriate training and organisational procedures.
• Strong communication and interpersonal skills.
• Ability to work independently and confidently within participants’ homes.
• Ability to work collaboratively and contribute positively to team culture.
• A commitment to person-centred practice, human rights, safeguarding, choice and independence.
• A reliable vehicle with comprehensive insurance, as there may be times when you are required to use your own vehicle.

Highly Desirable

• Certificate IV in Disability, Community Services, Individual Support or a related qualification.
• Experience supporting people with disability in an individual or in-home setting.
• Experience supporting participants with complex or individualised support needs.
• Experience supporting community access and participation.

EMPLOYMENT REQUIREMENTS

The successful applicant will be required to:

• Hold or obtain a current NDIS Worker Screening Check.
• Hold or obtain a current Working with Children Check.
• Hold or obtain a current National Criminal History Check (Police Check).
• Hold current First Aid and CPR certification.
• Hold a current Australian driver’s licence.
• Access to a reliable, registered vehicle with comprehensive insurance where the vehicle is required for authorised work-related travel.
• Complete all mandatory training requirements.
• Successfully complete pre-employment screening checks.
