Job summary

United Foundation seeks Home & Community Support Workers to deliver person-centred care to NDIS participants in their own homes. You will help clients live independently, follow care plans, and build meaningful routines while ensuring safety and dignity.

Ideal candidates have 6+ months' experience or a Cert III/IV in Aged Care, Home & Community, or Disability, plus hands-on personal care and strong communication. A driver’s licence and vehicle are highly regarded.

Qualifications

• Minimum 6 months' experience or Cert III/IV in Aged Care, Home & Community or Disability.
• Hands-on personal care including grooming, toileting and mobility support.
• Experience in in-home care and working with disability support.
• NDIS checks, First Aid, driver’s licence and insured vehicle are preferred.

Responsibilities

• Provide personalised care and support in participants' homes.
• Assist with grooming, toileting, meals and medications.
• Follow personalised care plans and report changes promptly.
• Offer companionship and emotional support while respecting privacy.
