Job Description

Concierge Disability Support Worker / Enrolled Nurse

Casual and contract opportunities | From $45.28 per hour | Salary Packaging available

About Us

At Pathway Talent, we are passionate about delivering high-quality, person-centred support that empowers people living with disability to lead fulfilling and independent lives. Our Concierge Support Model focuses on providing premium, tailored support services that place the participant's goals, choice, and control at the centre of everything we do.

We are seeking experienced, compassionate, and professional Disability Support Workers and Enrolled Nurses to join our growing casual pool which could lead to permanent work.

What is the Concierge Support Model?

Our Concierge Support Model goes beyond traditional disability support. We provide a premium, highly personalised service where support workers and nurses become trusted partners in helping participants achieve their goals and live their best lives.

As a Concierge Support Worker or Enrolled Nurse, you will:

  • Build and maintain professional, participant-focused relationships with a small group of participants.
  • Understand each participant's preferences, routines, goals, and aspirations.
  • Coordinate and facilitate supports that enhance independence and quality of life.
  • Act as a proactive problem solver, identifying opportunities to improve participant outcomes.
  • Provide a responsive and professional service that focuses on participant experience.
  • Collaborate closely with families, guardians, support coordinators, allied health professionals, and internal teams.
  • Support participants with personal care, daily routines, and activities of daily living while maintaining their dignity and choice.
  • Deliver support that is flexible, innovative, and tailored to individual needs rather than a "one size fits all" approach.

In this role, you are more than a support worker or Enrolled nurse. You are an advocate, mentor, coordinator, and trusted point of contact who helps participants navigate daily life with confidence, choice, and control.

The Ideal Candidate

  • Thinks beyond task-based care and focuses on outcomes.
  • Takes initiative and ownership.
  • Has exceptional communication and relationship-building skills.
  • Is solutions-focused and highly organised.
  • Understands the importance of delivering a premium support experience.
  • Is passionate about empowering people to achieve their goals and live independently.

Our Concierge Model is built on four key principles:

  • Personalised Support - Every participant is unique.
  • Relationship First - Trust and connection drive positive outcomes.
  • Proactive Service Delivery - Anticipating needs before they become challenges.
  • Exceptional Participant Experience - Delivering support that exceeds expectations.

This is an ideal role for Support Workers and Enrolled Nurses who want to move beyond simply completing shifts and instead become an integral part of a participant's journey and success.

Essential Requirements
  • Certificate III or IV in Individual Support, Disability, Community Services, or equivalent (Support Workers).
  • Diploma of Nursing and current AHPRA registration (Enrolled Nurses).
  • Current NDIS Worker Screening Check.
  • Current Working With Children Check.
  • Current First Aid and CPR Certificate.
  • National Police Check.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to lift 10kg unassisted.
  • Experience in Concierge model a bonus but not essential.
  • Great pay and benefits
Earn from $45.28 per hour and access to salary packaging to reduce your taxable income and increase your take home pay by up to $15,900, plus $2,650 for entertainment expenses. Enjoy novated leasing.
  • Flexible work on your terms
Choose when, where, and how you work.
  • Ongoing training and professional development
Grow through mentoring, skills training, and career development pathways.
  • Purpose above all else
Be part of a community that values compassion, connection, and wellbeing.
