NSW Health, Central Coast Local Health District, is seeking a Disability Pathway Navigator at Gosford Hospital to provide expert advice on NDIS processes and strengthen health-disability interfaces. This permanent full-time role offers 38 hours per week and remuneration commencing from $118,821 per annum.

You will develop pathways, lead system improvements, and work with internal and external stakeholders to improve access to disability and health services.

Competenze

• Experience in disability, health or community services with knowledge of NDIS.
• Ability to build collaborative relationships with diverse stakeholders.
• Strong analytical, problem-solving and project management skills.
• Experience developing policies, pathways and reporting frameworks.
• Excellent written and verbal communication for high-level advice.

Mansioni

• Provide specialist advice on NDIS processes to staff, consumers and stakeholders.
• Facilitate escalations and develop strategies to address barriers to care and access.
• Build partnerships to improve coordination and service delivery outcomes.
