Qualifications

• At least 12 months relevant work experience in community services or similar roles.
• Experience with case management, report preparation, and NDIS documentation processes.
• Experience working with vulnerable individuals, people with disabilities, or culturally diverse communities.
• Experience dealing with mental and emotional disturbances and developmental disability.
• Experience working with government agencies, non-profits, and other service providers.

Responsibilities

• Assess participants' needs, circumstances and goals and identify appropriate services.
• Coordinate services and referrals, liaise with families and service providers.
• Assist participants to access community programs and resources and support participation.
• Maintain accurate case notes, records and documentation; prepare progress reports.
• Collaborate with participants, families, colleagues and external stakeholders while safeguarding confidentiality.
