Job summary

Proveda is a major community-led organisation in the Northern Beaches providing impartial information and guidance to older people, those with dementia, disabilities and complex health needs. The Care Coordinator role supports navigating NDIS, MAC and health services, helping clients set goals, and coordinating with providers to ensure necessary care.

Qualifications

• Cert III or above qualifications in community services, disability, social work, mental health or related discipline.
• Experience in NDIS and/or Aged Care sectors.
• Demonstrated knowledge of service delivery systems including NDIS, MAC or NSW Health is preferred.

Responsibilities

• Provide impartial information, advice and guidance to people and families to navigate the NDIS, My Aged Care and health services.
• Support customers to set and achieve goals within their plan or package budget via phone and occasional visits.
• Connect customers with informal, mainstream and funded supports.
• Build capacity of customers and their families to manage their own care.
• Liaise with network of approved providers to ensure necessary care and advocate for changes when issues arise.
