Job summary

Empower Healthcare is seeking a Home Care Physiotherapist to deliver in-home assessments and tailored treatments across Sutherland Shire and St George. You will collaborate with a multidisciplinary team to optimise independence and community participation for NDIS participants.

Qualifications

• Bachelor of Physiotherapy (or equivalent) and registration with AHPRA.
• Police Check Clearance.
• Excellent verbal and written communication skills.
• Strong time-management skills and attention to detail.

Responsibilities

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home.
• Work holistically with other allied health professionals to provide a multidisciplinary care plan.
• Educate participants and their families to optimise their ability to live healthy lives within the community.
• Liaise with service providers to ensure effective communication.
• Work autonomously to prioritize and complete tasks, maintain records and provide patient reports.
