Job summary

Empower Healthcare in the Eastern Suburbs of Sydney is seeking a Home Care Physiotherapist to deliver high-quality care to participants in their own homes and via Support at Home/NDIS.

You will conduct assessments, develop tailored treatment plans, and work within a multidisciplinary team to promote independence and community participation. AHPRA registration and a Bachelor of Physiotherapy are required, with strong communication and time-management skills.

Qualifications

• Bachelor degree in Physiotherapy or equivalent required.
• Registration with AHPRA required.
• Police Check Clearance required.
• Excellent verbal and written communication skills.
• Strong time-management skills and attention to detail.

Responsibilities

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home through Support at home packages/NDIS.
• Work holistically with other allied health professionals to provide a multidisciplinary care plan
• Educate participants and their families to optimise their ability to live healthy lives within the community
• Liaise with service providers to ensure effective communication
• Work autonomously to prioritize and complete tasks, maintain records and provide patient reports
