Job summary

Southern NSW Local Health District invites applications for a MERIT Case Manager in Moruya. You will provide holistic case management and coordinate treatment options for individuals with complex needs within a multidisciplinary team.

Qualifications

• Tertiary qualifications and current registration with AHPR as RN, OT or Psychologist, or tertiary qualifications for Social Worker/Counsellor/Welfare Officer with eligibility for membership.
• Experience in holistic alcohol and other drugs treatment, including screening, assessment, brief intervention, withdrawal management, discharge planning, referral, group work and follow up.
• Applied knowledge of case management and treatment options for people and families with problematic drug use.

Responsibilities

• Develop and coordinate case management and treatment options for clients with complex needs.
• Collaborate with government and non-government services and community groups.
• Provide leadership within multi-disciplinary teams or autonomously in various settings.
• Demonstrate high-level communication and proficient computer skills.
• Maintain and travel as required, including long-distance travel and overnight stays.
