Job summary

Austin Health in Melbourne's north-east is seeking a Case Manager to work with the MSTS Consultant Psychiatrist and the team to provide comprehensive case management for individuals facing mental health challenges.

The role involves coordinating person-centered care, improving psychological and social well-being, and working in shifts that span weekdays and weekends. A flexible, collaborative approach and current registration in a health-related discipline are essential.

Qualifications

• Minimum qualification in a health-related field with current registration.
• Open to RPN Grade 3 / OT Grade 3 / SW Grade 3 / PSY Grade 2.
• Excellent written and verbal communication; ability to work autonomously and in a multidisciplinary team.

Responsibilities

• Undertaking assessment and case management of NEMSTS consumers and contributing to treatment objectives.
• Deliver high standard care for a clinical caseload and liaise with family and organisations.
• Provide assertive mobile outreach for consumers in the community.
• Medication monitoring for community consumers and planning interventions with the team.
