Grampians Health in Stawell invites applications for a Memory Support Nurse (fixed term, part time 16 hours per week) to join our Stawell Community Services. You will provide specialist dementia care, support clinical leadership, and coordinate person-centred care planning with consumers, families and health professionals.

Qualifications

Memory Support Nurse - Stawell Community Services

Fixed Term Part Time 16 hours per week (0.4 FTE) until July 2027

About the Role

Applications are invited for a Memory Support Nurse within our Stawell Community Services.

The Memory Support Nurse provides specialist care, clinical expertise, and leadership in the area of memory dementia care, working with consumers, carers, families and health care professionals to guide and coordinate person centred assessment and care planning and delivering high quality best practice care.

Key Responsibilities

• Conduct memory and cognitive assessments to support timely diagnosis and care planning.
• Develop and review person-centred care plans in partnership with consumers, families and carers.
• Provide specialist dementia care advice, education and support to consumers, carers and staff.
• Coordinate referrals and connect consumers with appropriate health and community services.
• Promote best-practice dementia care through clinical leadership, mentoring and advocacy.
• Collaborate with multidisciplinary teams to improve care outcomes and service integration.

Skills & Experience

• Registered Nurse with a current AHPRA registration.
• High knowledge of dementia and memory loss
• Evidence of recent Dementia Courses or Training attended
• Experience post-graduation preferably in Aged Care Settings or Community Settings
• Experience working in a community setting, aged care or acute settings where advanced achievement in clinical knowledge, clinical skills, problem solving and relevant experience for the role
• Demonstrate experience and expertise in dementia and dementia related behaviour
• A commitment to develop the role and increase awareness of dementia behaviour management across the organisation, including development of referral pathways
• Demonstrated ability to prioritise workload and work under pressure.
• Highly developed interpersonal communication skills and computer literacy skills.
• Self-motivated and able to work autonomously within a team environment.
• A current Australian Drivers Licence.
