Job summary

Colac Area Health is seeking a Lymphoedema and Wound Management Clinician to provide advanced clinical care for lymphoedema, chronic oedema and complex wounds in the Colac-Otway region.

The role is part-time (up to 16 hours per fortnight, 0.2 EFT) and requires expert clinical assessment, education and leadership to improve client outcomes and integrate care with multidisciplinary teams.

Qualifications

• Registration under AHPRA as a nurse or allied health professional.
• Specialist lymphoedema and complex wound care qualifications preferred.
• Current checks and licenses as listed in the requirements.

Responsibilities

• Provide advanced clinical assessment, treatment and education for lymphoedema and complex wounds.
• Lead service development, quality improvement and evidence-based practice.
• Collaborate with multidisciplinary teams to form integrated care models.
• Support patient self-management and reduce avoidable hospital admissions.

About the role

Lymphoedema and Wound Management Clinician

The Lymphoedema and Wound Management Clinician provides advanced clinical expertise in the assessment, treatment, education and management of clients with lymphoedema, chronic oedema and complex wounds within Colac Area Health and thewider Colac-Otway region.

The Lymphoedema and Wound Management Clinician is recognised as an expert clinician who provides consultancy, clinical leadership, service development, education, quality improvement and evidence-based practice to support optimal client outcomes. The role works collaboratively with multidisciplinary teams to develop integrated models of care, reduce preventable complications and improve client self-management.

The role delivers safe, evidence-based, person-centred clinical care requiringadvanced clinical reasoning and autonomous decision-making within scope of practice. The clinician contributes to improved client outcomes through early intervention, prevention, education, chronic disease management, and reduction of avoidable hospital admissions related to cellulitis, wounds, and chronic oedema complications.

The position also contributes to service development, staff education, quality improvement, multidisciplinary collaboration, and community-based integrated care in alignment with CAH strategic priorities.

Qualifications, Experience and Personal Attributes –

• Registered Nurse or Allied Health professional with Australian Practitioners Regulation Agency and holds current registration
• An Australasian Lymphology Association accredited post graduate qualification in complex lymphatic therapy to meet clinical accreditation requirements
• Post graduate qualification in advanced wound care or working towards completion of qualifications
• SWEP amber or red registered and credentialled practitioner
• Demonstrated clinical experience in lymphoedema, chronic oedema, complex wound care, rehabilitation, oncology, community nursing, or a related speciality area
• Demonstrated advanced assessment, clinical reasoning, and treatment planning skills
• Understanding of compression therapy principles and chronic oedema management
• Highly developed communication and interpersonal skills
• Ability to work autonomously
• Demonstrated understanding of multidisciplinary and person-centred care
• Computer literacy and ability to utilise clinical information systems
• Current Driver’s Licence
• Current Police Check
• Current Working With Children Check
• Membership of an appropriate professional association
