Job summary

Eloma Care is looking for a committed Lifestyle Assistant to help participants build confidence in daily routines and join community activities. You will collaborate with families to meet goals, maintain records, and support clients to express preferences respectfully.

Applicants should have NDIC/WWCC, first aid, driver’s license, and complete the NDIS induction. A caring team environment with growth opportunities awaits.

Qualifications

• Current NDIS Worker Screening and WWCC.
• First Aid Certificate.
• Driver's License.
• New Worker NDIS Induction Module completion required before commencing employment.
• Clear Police Check.
• Recommended qualifications: Certificate III in Individual Support, Mental Health, Aged Care, Community Services, Disability Work or similar.

Responsibilities

• Support daily living with purpose by helping participants build confidence and independence in their everyday routines
• Work within our day program to complete community access and activities
• Empower clients to make choices and build confidence in their own abilities
• Join participants in community activities, social programs, and outings like shopping or events
• Work closely with participants, their families, and key supports to understand their goals and preferences
• Maintain clear and accurate records and review support plans
• Help participants express their views, raise concerns, and feel heard and respected
• Identify and report any risks or concerns to your supervisor promptly
• Use approved, trained restrictive practices when required and always with care and compliance
