Job summary

Monash Health seeks a compassionate Leisure and Lifestyle Assistant for Chestnut Gardens Aged Care Home in Doveton. You will engage residents in meaningful activities, support wellbeing plans, and help run monthly calendars and outings to promote emotional and spiritual well-being.

You will work within a team, follow infection control and Aged Care Standards, and hold essential NDIS clearance, police checks, and a Certificate IV in Leisure and Health.

Qualifications

• Valid NDIS Clearance and completion of the NDIS Worker Orientation Module (essential)
• Valid police check clearance including a renewal every 3 years (essential)
• Certificate IV in Leisure and Health (CHC43415) or equivalent qualification (essential)
• Current Victorian Driver’s Licence (essential)

Responsibilities

• Plan and deliver individual and group activities that reflect residents’ interests, preferences, and goals
• Contribute to personalised wellbeing care plans and ensure timely documentation and evaluation
• Support the coordination and delivery of monthly activity calendars, special events, and community outings
• Provide emotional and spiritual support, especially during end-of-life care
• Uphold Monash Health iCARE values and adhere to infection control, OHS, and Aged Care Standards including SIRS and NDIS requirements
