BaptistCare NSW/ACT - Aged Care in Warragul is seeking a part-time Lifestyle Assistant to deliver engaging, person-centred lifestyle and recreational activities for residents. You will work in a multidisciplinary team, promoting independence, social connection and quality of life for older Australians.

Applicants should have relevant qualifications such as Certificate III in Individual Support (Ageing) or Leisure & Health, with experience in aged care or community services.

Qualifications

• Qualifications in Leisure & Health, Diversional Therapy, Lifestyle & Leisure or similar.
• Certificate III in IS Ageing or higher qualification.
• Experience in Aged Care, Community Care or Disability Support.
• NDIS Screening Clearance mandatory.

Responsabilités

• Deliver person-centred lifestyle and wellbeing programs.
• Facilitate individual and group activities to promote independence and social connection.
• Monitor, document and evaluate resident participation and outcomes.
• Build relationships with residents, families and the care team.
• Collaborate to deliver holistic, resident-centred care.
• Identify and report risks and safety concerns.
• Support events, cultural celebrations, and community engagement.
