Mallee Track Health & Community Service invites applications for a Leisure and Lifestyle Worker. The role focuses on creating and implementing activities to enhance quality of life by helping individuals engage in a range of leisure interests across community sites in Ouyen and beyond.

Applicants should have a Certificate IV in Leisure and Health or be willing to work towards it, possess a current driver’s licence and police check, and demonstrate high ethical standards while representing MTHCS.

Qualificações

• Certificate IV in Leisure and Health or willingness to gain qualification.
• Current Driver's Licence.
• Current Police Check.
• Demonstrated ethical standards and professional representation.

Responsabilidades

• Provide a recreation and lifestyle program to enhance wellbeing and independence across multiple locations.
• Complete comprehensive assessments documenting each consumer's needs, goals and preferences.
• Involve consumer and family in developing an individualised leisure and lifestyle program.
• Plan and coordinate leisure interests for people living with dementia and complex needs.
• Contribute to strategies to address behaviours and symptoms of dementia within a multidisciplinary team.
