Job summary

Central Coast Local Health District in NSW seeks a Clinical Change Manager to drive the implementation and optimisation of the Virtual Hospital Care model across the District. You will lead clinical transformation, engage stakeholders and coordinate complex projects across multidisciplinary teams.

The role requires expertise in contemporary care models, telehealth, and patient flow optimisation, with an emphasis on delivering safe, person-centred care and measurable benefits.

Qualifications

• Degree in a health-related discipline and relevant healthcare system knowledge.
• Experience in clinical transformation and service improvement initiatives.
• Knowledge of virtual care technologies and telehealth.
• Ability to coordinate complex projects with multidisciplinary teams.
• Strong communication, collaboration and stakeholder engagement skills.

Responsibilities

• Lead planning, coordination, implementation and evaluation of clinical transformation initiatives.
• Drive out-of-hospital care models and virtual care initiatives to improve patient outcomes.
• Coordinate project planning, engagement, co-design and implementation processes.
• Collaborate with clinical and operational leaders, gov NGOs, and community partners.
• Apply knowledge of telehealth and digitally enabled care models to adoption and optimisation.
• Develop and manage project plans, track milestones, monitor performance and risks.
• Monitor benefits realisation and support sustainability of initiatives.
• Provide high-level advice and reports to senior leaders.
