Job summary

NSW Health is seeking an experienced Clinical Change Manager for Virtual Hospital Care at Central Coast Local Health District. The role leads planning, delivery and evaluation of clinical transformation to support out-of-hospital and virtual care, improving patient flow and outcomes across the district.

You will collaborate with CCHaH and clinical teams to drive innovative care models, monitor benefits and report to senior leaders while ensuring safe, person-centred care delivery.

Qualifications

• Degree in a health-related discipline with knowledge of the healthcare system.
• Strong understanding of out-of-hospital care, HITH and virtual care models.
• Experience leading multidisciplinary clinical change, project management and change management strategies.
• Knowledge of virtual care technologies (telehealth, remote monitoring).
• Ability to coordinate planning, engage stakeholders, co-design and drive implementation.

Responsibilities

• Lead planning, coordination, implementation and evaluation of clinical transformation and service improvement initiatives.
• Drive out-of-hospital and virtual care initiatives to improve patient flow and outcomes.
• Coordinate project planning, stakeholder engagement, co-design and problem-solving for complex changes.
• Collaborate with multidisciplinary teams and external partners to redesign services and strengthen care pathways.
• Apply virtual care technologies to support adoption and optimization of care models.
• Develop project plans, track milestones, monitor performance and manage risks.
• Monitor benefits, pursue continuous improvement and ensure sustainability of initiatives.
• Provide high-level advice and progress reports to senior leaders aligning with organisational priorities.
