Job summary

Calvary Home Care - Darwin is seeking a Case Manager to lead a small team and manage a caseload under CHSP funding. You will coordinate care requirements, maintain budgets and onboard new clients, contributing to independence and quality of life for clients.

Qualifications

• Experience leading a fast-paced team.
• Demonstrated leadership in case management or senior role.
• Relevant tertiary qualification in the community sector or Cert4 in leadership desirable.
• Experience in the aged care, community or disability sectors with complex needs.
• Full driver's license and insured vehicle, smartphone.
• Knowledge of My Aged Care portal/assessment process.
• NDIS clearance or willingness to obtain.
• Police clearance within last 6 months or willingness to obtain.

Responsibilities

• Manage a caseload under CHSP/HCP funding.
• Onboard and grow new clients; manage care requirements.
• Maintain budgets and liaise with stakeholders.
• Lead a small team of various roles.
• Understand the Aged Care Standards.
