Calvary Community Care in Adelaide is seeking a dedicated Support at Home Case Manager to coordinate person-centred care for older Australians living at home. Based in the Adelaide CBD, the role blends office-based work with community visits, managing a caseload, developing care plans and liaising with families and providers.

You will ensure compliance with guidelines, document accurately and deliver compassionate, client-focused care, supported by company vehicles and standard business hours.

Qualifications

• Experience in case management and coordination.
• Understanding of the aged care program and guidelines.

Responsabilités

• Manage a caseload of clients, including assessments, care planning and reviews.
• Coordinate and monitor in-home services with clients, families and providers.
• Ensure compliance with guidelines and maintain accurate documentation.
