Calvary Home Care Adelaide is seeking a dedicated Case Manager to coordinate and deliver person-centred care for older Australians at home. Based in Adelaide CBD, this full-time role combines office work with community visits, supported by company vehicles and standard hours.

You will assess clients, develop tailored care plans, liaise with families and providers, and ensure documentation complies with guidelines while promoting independence and dignity.

Qualifikationen

• Understanding of the new aged care act and support at home program.
• Experience in case management and coordination.
• Full driver's licence.

Aufgaben

• Manage a caseload of clients, including assessments, care planning, and regular reviews to support independent living.
• Coordinate and monitor in-home services, working closely with clients, families, and service providers.
• Ensure compliance with Support at Home guidelines while maintaining accurate documentation and delivering person-centred care.

Kenntnisse

Understanding of aged care act

Ausbildung

Certificate III or IV in Aged Care

Certificate in Disability or Community Services

Diploma of Community Services (Case Management)
