Working as part of the District Nursing Service, you will provide person-centred, evidence-based nursing care to clients within the community setting. The role supports individuals to maintain health, independence and quality of life while receiving care in their own homes and community environments.
This is an excellent opportunity to develop your skills in community nursing while working within a supportive and experienced team committed to delivering high-quality care across the region.
Deliver high-quality clinical care including wound management, medication administration, assessment and ongoing care planning for clients with acute, chronic and complex healthcare needs.
Support patients and their families through collaborative care planning, education and health promotion activities designed to maximise independence and wellbeing.
Coordinate services and work closely with multidisciplinary teams to ensure continuity of care across community, hospital and aged care settings.
Work independently within the community setting, exercising sound clinical judgement and responding appropriately to changing patient needs.
Participate in a seven-day rotating roster, including weekends, public holidays and an on-call roster supporting palliative care clients.
