Job summary

Western District Health Service is inviting expressions of interest from current WDHS staff for a temporary District Nurse backfill to cover Long Service Leave, based in Hamilton, Victoria. The role involves delivering community nursing care to support independence and quality of life for clients in their homes and community settings.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Diploma or Bachelor of Nursing.
• Minimum 2-3 years post-graduate nursing experience.
• Demonstrated clinical assessment and care planning skills.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a community-based caseload.
• Commitment to person-centred care and continuous improvement.
• Current Driver's Licence.
• NDIS Worker Screening Check and Working with Children Check (or willingness to obtain).
• Rights to work in Australia.

Responsibilities

• Deliver high-quality clinical care including wound management, medication administration, assessment and ongoing care planning for clients with acute, chronic and complex healthcare needs.
• Support patients and their families through collaborative care planning, education and health promotion activities designed to maximise independence and wellbeing.
• Coordinate services and work closely with multidisciplinary teams to ensure continuity of care across community, hospital and aged care settings.
• Work independently within the community setting, exercising sound clinical judgement and responding appropriately to changing patient needs.
• Participate in a seven-day rotating roster, including weekends, public holidays and an on-call roster supporting palliative care clients.
