Job summary

Brophy Family & Youth Services is seeking a Casual Integrated Family Services Practitioner to support families, strengthen relationships and promote child and youth well-being within the community. You will deliver targeted care packages and work across programs with schools, health services and community organisations to provide practical, early intervention and tailored guidance.

The role focuses on building capacity and resilience, engaging with vulnerable families, and coordinating services

Qualifications

• Bachelor of Social Work or Diploma in Community Welfare Work (or equivalent) approved by professional bodies (AASW/APS).
• Current Drivers Licence.
• Experience with disadvantaged children/families and child protection/out-of-home care.

Responsibilities

• Undertake case management including intake, assessment, care planning, coordination, key worker role, brief interventions, referrals and case reviews.
• Establish and maintain action plans and outcome star plans for adults and families.
• Collaborate with housing offices, service providers and local agencies to ensure coordinated support.
• Develop professional relationships with a broad range of stakeholders.

About the Role

As our casual Integrated Family Services Practitioner, you’ll support families to strengthen relationships, build safety and stability, and help children and young people thrive. Working alongside families experiencing vulnerability, you’ll use a strengths-based, trauma-informed approach to support capacity building, resilience and positive change. In particular the delivery of Targeted Care Packages within the home and community.

This casual role offers flexibility and variety, with shifts across a range of programs including Adolescent Support, Finding Solutions, Family Services, Family Preservation & Reunification, Targeted Care Packages, and Who’s In Charge. You’ll provide practical support, early intervention and guidance that is tailored to each family’s goals and needs. In particular the delivery of Targeted Care Packages within the home and community.

Collaboration is a key part of the role. You’ll work closely with schools, health services, community organisations and other stakeholders to ensure families receive coordinated and well-rounded support.

• Undertake case management including intake, assessment, case/care planning, case coordination, key worker role, brief interventions, supported referrals, action plans, case reviews and case closure.
• Establish, facilitate and maintain action plans and outcome star plans for adults and families.
• Work in close partnership with local housing offices, other housing and support providers, Gunditjmara Aboriginal Cooperative, Southwest Health Care – Mental Health Services and Western Region Alcohol and Drug Centre and local government and departmental-funded public tenant groups.
• Establish appropriate community contacts and develop and maintain professional working relationships with a broad range of stakeholders.

Do your skills include:

• A sound understanding of issues confronting disadvantaged children, young people and their families, particularly clients involved with the child protection system and out of home care program.
• he a bility to make timely, sound decisions and assessments regarding
• Demonstrated knowledge and understanding of the appropriate legislation, case management practice, and the ability to adhere to policy and procedures.
• The a bility to maintain positive, effective working relationships with colleagues, peers, and relevant stakeholders in a wide range of government, business and community organisations.

Are your credentials:

• A minimum qualification of a sector recognised and accredited Bachelor of Social Work or Diploma in Community Welfare Work or equivalent. (Qualifications must be approved by recognised professional bodies: ie AASW, APS etc.)
• A current Drivers Licence.
