Job summary

My Guardian in Hobart, Tasmania invites applications for a Client Intake Officer. The role focuses on face-to-face engagement with prospective clients and families, guiding them through intake, funding, and service options within Aged Care and NDIS packages.

You will collaborate with the TAS BDM and Case Management teams to ensure smooth onboarding, accurate CRM records, and high-quality customer service across Tasmania.

Qualifications

• Minimum 1 year of client intake, case management, or customer-facing experience.
• Knowledge of Aged Care (Support at Home / Home Care Packages) funding and NDIS frameworks.
• Excellent communication and empathy in client interactions.

Responsibilities

• Engage with prospective clients face-to-face to assess needs and recommend care solutions.
• Guide clients through intake and funding options for SaH packages and NDIS services.
• Collaborate with TAS BDM and Case Management teams for onboarding and ongoing care delivery.
• Generate and manage local referral pipelines and coordinate documentation and service transitions.
• Communicate funding levels, fees, and service inclusions during consultations.
• Maintain CRM records and provide regional reporting on client acquisition and conversion.
• Coordinate with rostering and clinical teams to align services with care plans.
