Job summary

My Guardian is seeking a motivated Client Intake Officer for our Queensland office. You will be the primary contact for prospective clients and families, guiding them through intake and decision-making with empathy and professionalism.

In this fast-paced, relationship-driven role you will engage face-to-face, schedule meetings, discuss Support at Home packages and NDIS services, and collaborate with the BDM and Case Management teams to ensure a smooth onboarding and ongoing care delivery.

Qualifications

• Face-to-face engagement with clients and families.
• Experience with Support at Home (SaH) packages and NDIS services.
• Experience in intake, case management, or customer-facing roles.
• Knowledge of Aged Care funding and NDIS frameworks.

Responsibilities

• Engage with prospective clients through face-to-face meetings, phone, and email to assess needs and recommend care solutions.
• Guide clients on SaH Packages and NDIS services and convert enquiries into active clients.
• Collaborate with QLD BDMs and Case Management for ongoing care delivery.
• Coordinate onboarding, referrals, documentation, and waitlist management.
• Communicate funding levels, fees, and service inclusions during consultations.
• Maintain CRM records and report on regional client acquisition and conversion.
• Support rostering and clinical teams to deliver services per care plans.
