Job summary

UnitingCare Queensland is seeking an Intake Practitioner on a casual basis to support the Gold Coast Homelessness Service. You will respond to walk-ins, phone and third-party referrals, conducting intake, triage and risk assessment to link clients with housing and case management.

Qualifications

• Minimum Cert. IV in human services or related field.
• Current Drivers Licence and Positive Notice blue card.
• Proficiency in Microsoft Office (Word, Excel, Outlook).
• Experience in the community services sector.
• Ability to work with people in crisis and diverse groups.

Responsibilities

• Serve as first point of contact for intake via walk-in, phone and third parties.
• Conduct screening, triage and risk assessments and refer to case management.
• Set up Gold Coast Homeless Hub drop-in service daily.
• Capture data in SHIP and QHIP client systems.
• Coordinate access to drop-in services like meals, showers and laundry.
• Build rapport with individuals and families in crisis and provide support.
