Job summary

Wellways Australia is seeking an Intake Worker to join Connected Care in Northern Tasmania. You will be the first contact for people accessing our services, supporting intake, screening and assessment using the IAR-DST tool to understand needs and ensure timely access to care.

You will work within a statewide mental health service delivered by Wellways in partnership with Stride Mental Health and Clarity Health Care, providing recovery-oriented, person-centred support.

Qualifications

• Qualification in Mental Health, Community Services, Human Services or Social Work at Certificate IV level.
• Experience undertaking intake and assessment to identify referral pathways.
• Recovery-oriented and trauma-informed practice understanding.
• Commitment to culturally safe, inclusive and person-centred practice.
• Ability to build professional relationships with referrers and stakeholders.
• Right to work in Australia; valid driver's licence; willingness to travel.
• Satisfactory National Police Check; International Police Check if applicable.
• NDIS Worker Orientation Module Certificate – Quality, Safety and You – online course.

Responsibilities

• Provide intake, screening and assessment support for participants.
• Identify referral pathways based on individual needs.
• Escalate clinical or safety concerns per organisational procedures.
• Collaborate with peer workers and multidisciplinary team to coordinate care.
