Job summary

Wellways Connected Care in Northwest Tasmania seeks a welcoming Intake Worker to be the first contact for people accessing our services. You will support intake, screening and assessment to understand needs and enable timely access to care, working with a multidisciplinary team.

You will apply trauma-informed, recovery-oriented approaches, promote informed choice and cultural safety, and coordinate with partner organisations to ensure appropriate referrals and safe escalation when needed.

Qualifications

• Experience undertaking intake, screening and assessment to identify referral pathways.
• Recovery-oriented and trauma-informed practice understanding.
• Commitment to culturally safe, inclusive and person-centred practice.

Responsibilities

• Provide intake support and assess participant needs for care options.
• Identify referral pathways and coordinate with multidisciplinary team.
• Escalate safety or clinical concerns per procedures.
