Job summary

ATSICHS Aboriginal and Torres Strait Islander Community Health Service Brisbane in Brisbane, QLD, is seeking a Social Health Care Coordinator to assess, triage and refer clients within a culturally safe framework. The role coordinates intake, screens needs, and delivers holistic social and wellbeing supports including case management for individuals and families.

Qualifications

• Bachelor qualification in relevant field as listed in job requirements.
• Ability to apply a high level of practice judgement when working with clients.
• Ability to work with diverse clients using a culturally responsive framework.

Responsibilities

• Provide initial engagement with families/individuals and screen suitability for Social Health services.
• Deliver individual case management sessions and coordinate referrals to appropriate services.
• Support medical staff by identifying referrals and addressing social-emotional wellbeing issues.
• Work as part of a multi-disciplinary team including Psychologists, Counsellors, GPs, Aboriginal Health Workers and Allied Health staff.
• Maintain accurate and consistent case notes.

Bachelor qualification in Social Work / Human Services / Behavioural Science / Social Science / Psychology / Criminology / Allied Health / AOD / Occupational Therapy / Sociology / Public Health
