Aboriginal & Torres Strait Islander Community Health Service Brisbane is seeking a Social Health Care Coordinator. You will assess and triage clients, coordinate intake, and deliver case management and social and wellbeing supports in a strengths-based framework.

You will work within a multidisciplinary team, ensuring culturally safe, holistic care and timely referrals, with a focus on SEWB supports and inclusive service delivery.

Competenze

• Demonstrated ability to apply a high level of practice judgement when working with clients.
• Ability to work with a culturally and socially diverse group of clients, applying a culturally responsive framework.
• Experience in providing case managed SEWB supports.
• Ability to record accurate and consistent case notes.

Mansioni

• Provide first point of engagement with families and individuals in a welcoming manner and assess suitability for Social Health services.
• Clients are provided with individual case management sessions.
• Manage clients during crises and de-escalate towards sustainable support.
• Collaborate with medical staff to identify referrals and manage social-emotional wellbeing issues.
• Work as part of a multi-disciplinary team including psychologists, counsellors, GPs and Aboriginal health workers.
