Job summary

AnglicareSA is seeking an Experienced Enrolled Nurse to join our Community Aged Care team as an Assessment Partner in the Adelaide region. You will perform in-home assessments, guide care planning and support older adults to maintain independence at home.

With a minimum of two years working autonomously and current AHPRA registration, you will collaborate with a multidisciplinary team to deliver person-centred care and sound clinical advice for complex needs.

Qualifications

• Tertiary qualification in Nursing, Allied Health or related field is required.
• Current AHPRA registration (where applicable) is required.
• Experience in community aged care or complex care settings is preferred.
• Strong assessment, care planning and clinical decision-making skills are essential.

Responsibilities

• Conducting in-home assessments and reviews to guide care planning.
• Coordinating your calendar and managing daily visits across the community.
• Supporting person-centered care focused on independence and re-enablement.
• Providing sound clinical advice and guidance on complex or escalated care needs.
