Job summary

Western District Health Service – Hamilton VIC is seeking compassionate Community Support Workers – Personal Care to join our Community Services team. You will provide person-centred in-home personal care, domestic assistance and social support to participants living in their own homes across the Southern Grampians and surrounding regions.

Flexible permanent part-time and casual positions are available with minimum 15 hours per week up to 38 hours.

Qualifications

Responsibilities

• Provide in-home personal care services including bathing, grooming and continence support.
• Provide domestic assistance such as cleaning, laundry and cooking as part of daily living support.
• Assist participants with shopping, meal preparation and transport where required.
• Monitor wellbeing and report changes or concerns to the team.
• Collaborate with families, carers and the Community Services team to ensure quality care.
